In this vital role, you will serve as a central point of contact for patients, healthcare providers, and support staff, ensuring seamless coordination of care services. Your proactive approach will help improve patient experiences, streamline communication, and facilitate access to necessary medical resources. Responsibilities Act as the primary liaison between patients, medical providers, and support teams to coordinate appointments, treatments, and follow-up care Assist patients with scheduling visits and managing their treatment plans effectively Maintain accurate and up-to-date medical records, ensuring confidentiality and compliance with privacy regulations Provide administrative support within the medical office, including handling paperwork, development of care plans Communicate clearly with patients regarding their care instructions and upcoming appointments Collaborate with dental or other specialty offices to facilitate integrated patient care when applicable Support the medical team by preparing documentation, managing referrals, and tracking patient progress Managing care team training and scheduling Requirements Strong knowledge of medical terminology to accurately interpret clinical information Proven ability to provide excellent medical administrative support and customer service Excellent communication skills to effectively interact with diverse patient populations and team members Experience in assisted living, personal care, long-term care, or senior living environments preferred. Knowledge of state regulations, medication administration, care planning, and documentation requirements. Ability to work independently and manage multiple priorities in a fast-paced environment.
