Job Description
Position Overview The ID/DD Coordinator is responsible and accountable for the coordination and administration of services for clients and their caregivers under the direct supervision of the Location or Executive Director. Essential Job Functions
Provide initial evaluation to determine appropriateness and safety of client and environment.
Follow policies and procedures and conforms to all applicable patient care standards, licensure regulations and state laws
Provide a written summary for individual clients and services based on needs, goals, and expected outcomes in conjunction with the ISP to Supports Coordinators/Administrative Entities on a monthly basis
Participate in the development of Individual Support Plan
Maintain patient confidentiality
Demonstrate an understanding of client's medical status and updates self on clients/family status as needed or requested
Maintain effective communication between staff, supports coordinators, and family
Review progress notes from paraprofessionals to determine appropriate service delivery
Educate paraprofessionals on individual client ISP expectations
Perform on site evaluation of employee performance at 90 days and annually Requirements
Bachelor's degree in Psychology or other social science or related field
Current CPR certification
Proof of eligibility to work in the United States
Current completion of child abuse clearance and Fingerprinting Additional state specific requirements

Must be able to speak, write, read and understand English
Must be able to travel; company does not provide vehicles or transportation
Frequent lifting, carrying, pushing and pulling of 25 pounds
Occasional lifting, carrying, pushing and pulling of 50 pounds

Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions
