Job Description

We are seeking an energetic, highly motivated, and results-driven Clinical Hospice Liaison to serve as a key patient advocate within acute care settings. In this role, you will partner with hospital teams to support patients and families through care transitions, ensuring timely access to appropriate services including hospice, palliative care, and home health. The Hospice Liaison - Clinical plays a critical role in clinical assessment, care coordination, and relationship management , while supporting referral growth through strong partnerships with case managers, discharge planners, and social workers. https://www.youtube.com/watch?v=DyEH9buz_Vc KeystoneCare is an Equal Opportunity Employer Primary Job Resposibilities Serve as the primary clinical liaison between assigned hospital partners and the organization Build and maintain strong professional relationships with case managers, discharge planners, physicians, and social workers Conduct clinical evaluations of patients to determine eligibility for hospice and related services Collaborate with care teams to ensure smooth and timely transitions from hospital to home or appropriate care setting Educate patients and families on hospice, palliative care, and support service options , ensuring informed decision-making Review and interpret medical records to confirm clinical appropriateness and compliance with eligibility criteria Maintain regular on-site presence at assigned facilities and remain accessible for real-time coordination Support growth by facilitating referrals and strengthening provider relationships (non-traditional sales focus) Stay informed on hospital processes, patient needs, and industry trends impacting care transitions
Requirements Job Requirements:
Valid RN license with the state of Pennsylvania OR LCSW degree 3+ years experience as a clinical care coordinator, liaison, or case manager preferred Self motivated with excellent time management and communication skills Must have clinical knowledge of disease entities common to Home Health and Hospice Must have a valid Driver's License and a reliable vehicle Benefits Accrued Paid Time Off Paid Holidays Medical, Dental, Vision, and Aflac plans available $25K Company Paid Life Insurance Policy Retirement plan with discretionary employer match
