https://www.youtube.com/watch?v=DyEH9buz_Vc KeystoneCare is an Equal Opportunity Employer Primary Job Resposibilities Serve as the primary clinical liaison between assigned hospital partners and the organization Build and maintain strong professional relationships with case managers, discharge planners, physicians, and social workers Conduct clinical evaluations of patients to determine eligibility for hospice and related services Collaborate with care teams to ensure smooth and timely transitions from hospital to home or appropriate care setting Educate patients and families on hospice, palliative care, and support service options , ensuring informed decision-making Review and interpret medical records to confirm clinical appropriateness and compliance with eligibility criteria Maintain regular on-site presence at assigned facilities and remain accessible for real-time coordination Support growth by facilitating referrals and strengthening provider relationships (non-traditional sales focus) Stay informed on hospital processes, patient needs, and industry trends impacting care transitions

Requirements Job Requirements:
