Memory Care Coordinator Responsibilities:

Develop and implement individualized care plans for residents with cognitive impairments, in collaboration with the interdisciplinary team Create and maintain a calming and engaging environment, incorporating activities and therapies designed to stimulate cognitive function and emotional well-being Collaborate with family members and caregivers to provide education and support, addressing their concerns and promoting a sense of inclusion and involvement Document resident progress and care plans, ensuring accurate and timely communication with the healthcare team Participate in quality improvement initiatives, implementing evidence-based practices to enhance resident outcomes and satisfaction Maintain a safe and organized environment, adhering to established standards and protocols

Memory Care Coordinator Requirements:

Prior experience in a healthcare setting, preferably in gerontology or dementia care Possess strong communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare professionals Proficiency in electronic medical records and Microsoft Office applications
