Referral Specialist Salary Range:

This position is Monday - Friday, 8:30- 4:30 PM or 9:00-5:00 PM and located at the Stephen Klein Wellness Center, 2144 Cecil B. Moore, Philadelphia, PA. What You'll Do As a Referral Specialist , you will play a key role in ensuring patients receive timely, coordinated care. Manage referrals from provider orders through specialist follow-up, ensuring continuity of care Assist uninsured or underinsured patients with insurance applications and financial documentation Track and follow up on hospital admissions and referrals to support patient outcomes Answer incoming calls, schedule appointments, and support front desk operations as needed Provide clerical and administrative support to ensure a smooth clinic workflow Collaborate with care teams to meet Patient-Centered Medical Home standards What You'll Receive We take care of the people who take care of others.
