Description:
Position Purpose The Care Coordinator is responsible for coordinating services, support, and resources to ensure individuals receive high‑quality, person‑centered care. This role serves as a key liaison among individuals, internal teams, and external providers to promote continuity of care, safety, and positive outcomes.
What will you do:
Serve as the primary point of contact for care-related communication and problem-solving Facilitate referrals and connections to community resources, medical providers, and support services Monitor service delivery, identify gaps, and recommend adjustments as needed Coordinate resident admissions, orientations, and discharges in compliance with state, county, and organizational requirements. Advocate for individuals to ensure their needs, preferences, and rights are respected Respond to concerns or incidents and support resolution according to policy Collaborate with supervisors and leadership to support program goals and outcomes Minimum Qualifications Bachelor's degree and 3 years of related experience OR associate's degree and 4 years of related experience OR a High School Diploma and 9 years of related experience. Valid Driver's license and Ability to operate an organizational vehicle Act 33/34/73 clearances Pre-employment drug screen Knowledge of community resources, service systems, and care planning processes Strong organizational, communication, and problem-solving skills Ability to work independently and manage multiple priorities Proficiency with documentation systems and basic computer applications

SCHEDULE
This is a Non-Exempt (hourly) position. Primarily Monday through Friday daylight (8am-4pm) with flexibility to meet resident needs; some evenings, weekends, and holidays may be required.
