Seeking a Primary Care Family Nurse Practitioner (FNP) to support a Community Health Center dedicated to improving access to care in York County. Provide continuity of care for an existing and growing patient population. Manage and support an established patient panel, including individuals receiving Medication-Assisted Treatment (MAT). Help establish care for a vulnerable and underserved patient population in a mission-driven outpatient setting. Deliver both in-person and virtual primary care services, supporting flexible access for patients. Collaborate within a supportive care team consisting of two CRNPs and one physician. Practice in a team-based, integrated primary care environment focused on patient-centered outcomes.
