Part-time role (20-25 hours/week) Availability from 8-5 PM Remote Planned start date: Fall 2026 Core Responsibilities Manage a panel of assigned concierge patients Coordinate specialist referrals and follow-up appointments Conduct patient check-ins via phone, portal, and in-person Request, receive, and route medical records Document care plans and patient interactions in the EHR Triage and escalate urgent concerns appropriately Communicate with patients' broader care teams Qualifications Clinical background:

RN, LPN, NP, PA

with strong clinical experience 2+ years in primary care, care management, or concierge medicine Proficient with EHR systems Excellent written and verbal communication Highly organized; able to manage multiple patients independently Experience in concierge or direct primary care Comfort with high-touch, high-expectation patient relationships

Pay:

$45,000.00 per year
