Job Description

Sarge's Veteran Support:
Our mission is to heal the wounds, to restore dignity, hope and self-sufficiency to all homeless Veterans in need and to significantly reduce homelessness and poverty among our local Veterans. We accomplish this by operating transitional housing, family housing and permanent supportive housing for Veterans.

Sequim Washington Synopsis:
The Care Coordinator provides paraprofessional crisis intervention and short-term solution focused community-based advocacy services for and with the six elderly Veterans living at Minsky Place in Sequim. The Care Coordinator is the primary liaison with other public and private agencies providing services to the Veterans and supports all of Sarge's activities to include but not limited to resource development, community outreach and education and providing logistical support to activities and events.
Employment Preference :
Veterans, Veteran/Active-Duty will be given preference in the screening and selection process.
Schedule & Compensation:
This position operates on a mission driven schedule with flexibility and a commitment to work-life balance [four-day work week with some remote flexibility]. A typical schedule would be a 4/10 that alters on a biweekly basis. Hours are variable and negotiated through the selection process. Part-time is guaranteed, full-time is negotiable. Compensation is based on an experience and education evaluation with the typical range between $24-$30 dollars per hour.
Reporting Relationships:
This position is supervised by the Housing Manager.
Ideal Candidate:
The ideal candidate would be a veteran, a veteran/active duty dependent who has experience working with the elderly and case management. Some type of medical experience and a bachelor's degree in a related field would be positive attributes as well.

Required:
  • A citizen of or the ability to work in a legal capacity in the United States.
  • No serious involvement with the criminal justice system as determined on an individual basis but will not include any violent, sexual assault or drug offenses.
  • Veteran candidates must be in good standing [an honorable or general discharge from military services].
  • A minimum of two years of sobriety.
  • At least 21 years old, valid state driver's license with adequate active insurance and an acceptable driving record (as determined by third party insurance agent).
  • The completion of a successful background check and potential substance use screening.
  • A combination of knowledge skills and abilities that are reflective of the position duties and responsibilities.
  • Ability and experience in relating to and working with and on behalf of people from diverse backgrounds and income levels.
  • The ability to wear many hats and change them often.
  • The ability to maintain a professional demeanor in all situations and interactions while showing respect and dignity to all people.
  • A basic knowledge of military culture.
  • A basic knowledge of working with Seniors and some type of affiliation or affinity to them.
  • A working knowledge of community resources and medical processes.
  • The ability to multitask and prioritize.
  • Mastery of the English language, both verbal and written formats.
  • Intermediate computer and technology skills.
  • Proven critical thinking, problem solving and communication skills.
  • The ability to take direction while relying on self-motivation & management and independent thought and behaviors.
  • The ability to demonstrate strong work ethics and work effectively as a part of a team or independently.
  • The ability to adapt and overcome in providing support in a flexible manner to all sarge's activities and events.
  • The ability to understand the working culture at sarge's and to adapt to it to ensure maximum flexibility and effectiveness.
Preferred:
  • Bachelor's degree or above in a related field, four years of experience serving veterans and/or the elderly or any combination of the above that makes the candidate qualified for the position.
  • A member of the local or surrounding communities with a knowledge of community and medical resources and knowledge and experience in accessing these resources.
  • A person that has preexisting relationships with veterans' programs and resources.
  • Someone with related "lived experiences"
  • A superior communicator, problem solver and staunch advocate.
  • Someone with a basic knowledge of the human condition and services models that address challenges.
  • Someone with peer counseling certifications.
Duties and Responsibilities:
  • Actively participate in a transfer meeting with other coordinators or agency staff.
  • Verifying Veteran's eligibility.
  • Facilitating resident turnover and welcome by preparing bedrooms for all new residents entering (i.e. clean linens, dresser drawers, bedroom cleanliness etc.) and providing a house orientation.
  • Reviewing program and house rules with all new residents and ensure client compliance with house rules.
  • Ensuring that each veteran is interviewed, assessed, and begins case management within 48 hours of their move-in.
  • Conducting assessments and interviews with clients to determine eligibility, housing stability, mental health, substance abuse, health, education, income, employment, etc., and making appropriate and timely referrals.
  • Providing consistent limits and expectations regarding program policies and goal achievement.
  • Monitoring compliance with goals through regular contact individual and group connection.
  • Following up with referrals to ensure that veterans receive and access necessary services in a timely manner.
  • Creating a sense of community within Minsky House and an attachment/investment in the greater community.
  • Developing strategies for engaging with clients, creating awareness about the program through community outreach and education.
  • Attending and being an active participant in meetings at relevant agencies and providers across the geographic region.
  • Responding to emergencies after hours as needed.
  • Providing transportation (company vehicle) to necessary appointments and activities.
  • Acting as a standby/ witness to medical appointments as an appointed listener as invited or required.
  • Maintaining inventory of all household supplies (i.e. cleaning supplies, cooking utensils, furniture, household keys, etc.) and ordering supplies through the main office.
  • Monitoring security system and facilitating maintenance as needed.
  • Documenting and coordinating damage or needed repairs with the main office.
  • Tracking, monitoring and requesting routine maintenance and repairs through the main office.
  • Other duties as required.
Pay:
$24.00 - $30.00 per hour
