  • Full-time We are looking for a skilled and compassionate Care Coordinator to join a physician-led team serving patients in a skilled nursing facility setting in midtown Savannah.
This is a hybrid role — mornings are on-site at the facility, afternoons are remote. We are looking to fill this position immediately. About the Role The Care Coordinator is the connective tissue between patients, providers, and the clinical team. You will manage a panel of patients with complex, chronic conditions — conducting assessments, collaborating on care plans, coordinating transitions of care, and ensuring nothing falls through the cracks. This is a high-ownership role that requires someone who can operate independently, communicate clearly across teams, and advocate effectively for patients. What You'll Be Doing You will deliver chronic care management services to patients with complex or high-risk conditions, conduct assessments using standardized tools, collaborate with providers and specialists on individualized care plans, coordinate post-hospital follow-up and medication reconciliation, provide patient and family education, and document all care management activity accurately. You will also contribute to quality improvement initiatives and help prioritize patients for daily medical services. What We're Looking For A Licensed Practical Nurse (LPN) or Certified/Registered Medical Assistant (CMA/RMA) with at least 1-2 years of experience in chronic care management, primary care, ambulatory care, skilled nursing, or a hospital setting. Strong communicators who can hold their own with physicians, engage patients with empathy, and stay organized across a busy caseload. Experience with EHR documentation is preferred. An associate's degree or higher in a clinical or health-related field is a plus. Schedule & Compensation Monday through Friday. No weekends. Mornings are on-site at the facility. Afternoons are remote — but this is still a full-time, 9-to-5 position. You are expected to be available, responsive, and working throughout the entire day. The hybrid structure is about location, not a reduced workload or flexible hours. Hourly pay commensurate with credentials and experience. Set schedule with no weekend requirements.
