The Care Coordinator works closely with participants, healthcare providers, behavioral health providers, community organizations, and family supports to improve health outcomes, reduce barriers to care, and support overall wellness. The Care Coordinator uses a person-centered, trauma-informed, culturally responsive, and strengths-based approach to assist participants in navigating medical, behavioral health, social service, and community support systems. Essential Duties and ResponsibilitiesCare Coordination & Participant Support Conduct outreach and engagement activities with assigned participants. Complete participant intake, consent forms, assessments, and Health Action Plans (HAPs). Develop individualized care coordination plans based on participant needs and goals. Coordinate medical, behavioral health, housing, transportation, food access, and social service referrals. Monitor participant progress and maintain regular follow-up contact. Assist participants with scheduling appointments and accessing community resources. Support transitions of care following hospitalizations, emergency room visits, or other critical events. Promote participant self-management, wellness, and independence. Documentation & Compliance Maintain accurate, timely, and complete documentation in electronic health records and required platforms. Complete monthly encounters and documentation in compliance with Medicaid and Health Home requirements. Ensure all participant records, consents, and assessments meet compliance standards. Upload required documentation within organizational timelines. Participate in chart audits, quality improvement activities, and compliance reviews. Collaboration & Communication Collaborate with healthcare providers, behavioral health agencies, hospitals, Managed Care Organizations (MCOs), and community partners. Participate in interdisciplinary team meetings and case conferences. Maintain professional communication with participants, families, and partner organizations. Escalate urgent participant concerns or safety risks appropriately. Community Outreach Support outreach activities and community engagement initiatives. Build relationships with community organizations and referral partners. Assist with participant education and resource awareness. Required Qualifications High school diploma or GED required. Bachelor's degree in social work, psychology, public health, healthcare administration, human services, or related field preferred. Experience working with Medicaid populations, behavioral health, social services, or community-based programs preferred. Strong organizational, communication, and documentation skills. Ability to work independently and manage a caseload effectively. Knowledge of trauma-informed care, cultural humility, and participant-centered practices. Basic computer proficiency, including Microsoft Office and electronic documentation systems. Preferred Qualifications Experience in care coordination, case management, behavioral health, or healthcare navigation. Experience working with diverse communities, immigrant populations, or underserved populations. Bilingual or multilingual abilities strongly preferred. Knowledge of Washington State Health Home Program requirements preferred. Physical Requirements Ability to travel locally for participant visits, meetings, and community outreach. Ability to sit, stand, walk, and use office equipment for extended periods. Ability to lift up to 20 pounds occasionally. Work Environment Hybrid office and community-based work environment. Some evening or weekend flexibility may be required depending on participant needs.
