For more than 35 years, the PINE physician practice has provided advanced treatment of kidney and endocrine diseases. The practice has over 33 providers across more than 14 convenient office locations in Allegheny and Washington counties. Job Summary The Care Coordinator is a Certified Medical Assistant (CMA) with a strong nephrology background and/or outpatient clinical experience as a Patient Care Technician (PCT) or Certified Clinical Hemodialysis Technician (CCHT). This non-exempt position supports nephrologists, endocrinologists, and advanced practitioners in the care of patients with Chronic Kidney Disease (CKD) through a combination of clinical and administrative responsibilities. Key duties include gathering and organizing patient information, identifying unmet patient needs, engaging patients in their care, supporting treatment interventions, and facilitating communication between patients and their care teams. The Care Coordinator plays an integral role in delivering high-value, patient-centered care that improves access to preventive services and chronic disease management. Care Coordinators are expected to manage a caseload of approximately 500 patients while meeting established clinical and administrative performance metrics. Key Job Responsibilities Develops a keen understanding of our nephrology practice, requirements for optimal, coordinated population health. Works as an effective team member of the care team. Collaborates with care teams to establish population-appropriate, pre-visit, and point of care processes. Works with the Clinical team and Chronic Care Management platform to support patients with chronic disease and assists in coordination of the patient's care continuum. Contributes to quality improvement and care redesign of population health efforts. Manages patient registries. Provides the members of health care teams in designated practices with the data required to meet the health needs of the patient. Supports practice staff to develop interventions to proactively manage target populations. Contributes to a positive experience for patients and families through courteous telephone and digital interactions, accurate and expeditious routing, as well as referrals to appropriate clinical staff when necessary. Recognizes and reports data inconsistencies to appropriate personnel. Regularly attends and participates in meetings with coworkers and practice staff. Complies with OSHA, HIPAA, and other regulatory requirements, as well as applicable practice and broader Innocura policies and procedures. Knowledge, Skills, and Abilities High School Diploma Vocational/Technical School Certificate Degree Experience in a minimum of one of the following is required: o Nephrology office experience o Endocrinology office experience 0-2 years' experience as a Medical Assistant supporting a Nephrology or Endocrinology provider and/or
PCT/CCHT
