Job Description

Waukesha County, WI Full-Time | M-F, First Shift | Remote Flexibility $23.70/hr Lutheran Social Services of Wisconsin & Upper Michigan is seeking a compassionate and organized Care Coordinator to join our CLTS Waiver Program team in Waukesha County ! The Children's Long-Term Support (CLTS) Waiver Program is a federally funded Medicaid initiative that helps children with developmental, physical, or severe emotional disabilities receive services that allow them to thrive in home and community settings. As a Care Coordinator, you'll be the primary point of contact for families, helping them access and navigate essential services.

Assess children's functional abilities using approved tools Develop and implement individualized service plans with families and providers Facilitate team meetings and coordinate services based on family-centered goals Maintain accurate documentation and meet all regulatory timelines Collaborate with internal teams and external agencies to support families Testify in legal proceedings when required Communicate clearly with families, providers, and team members Participate in staff development, training, and supervision Medical, Dental & Vision Insurance Flex Spending (Health & Dependent Care) Mileage Reimbursement Paid Time Off + 10 Paid Holidays 403B Retirement Contribution Employee Assistance Program Service Awards & Recognition Remote Work Perks 1 remote day/week at 6 months 2 remote days/week at 9 months 3 remote days/week at 12 months 1 remote day/week at 6 months 2 remote days/week at 9 months 3 remote days/week at 12 months Bachelor's degree in a human services field (e.g., Social Work, Psychology, Special Education, Counseling, etc.) Minimum 1 year of experience working with children with disabilities Bilingual fluency in Spanish is preferred but not required Proficient in computer systems and electronic health records Strong interpersonal and organizational skills Valid driver's license and reliable transportation (MVR check required) Community-based with daily travel required Moderate noise level; occasional exposure to outdoor conditions Flexibility to meet family needs, including crisis response Ready to make a difference in the lives of children and families?
