Purpose:
NovumHealth is seeking a Community Care Coordinator who works within a healthcare setting to provide comprehensive care coordination services to patients, ensuring their healthcare needs are effectively managed and coordinated. This role plays a vital part in enhancing outcomes, promoting and optimizing healthcare resources, and ensuring the client's continuity of care needs are in place. This position will be required to drive between different locations in Polk County and other surrounding counties. As such, access to reliable transportation is required which can safely get the employee and members from one location to another in under an hour. Mileage will be reimbursed at the IRS rate.
Key Responsibilities:
(but not limited to) Meeting with members in person within their community to secure needed behavioral health treatment and support services. Coordinating the integrated care management needs of the member with the individual's outpatient provider, hospital, PCP and others involved in his/her care. Visiting members in person when admitted to a psychiatric hospital setting to assess needs, assist with discharge plans and coordinate with doctors, nurses, social workers and discharge planners Flexibility to attend multiple treatment settings same day to collect information and proactively monitor member's progress towards treatment goals/discharge. Participating in Care Management/Treatment Team Meetings Uphold the organization's ethics, code of conduct and customer services standards.
Knowledge, Skills and Abilities:
Knowledge of treatment plans. Good organizational and time management skills Ability to write clear, concise documentation for record-keeping and billing purposes Strong problem solving and communications skills (verbal and written) Excellent crisis management skills Ability to use electronic communications and internet resources.
Education, Licenses, Certifications and Experience:
Minimum of 2 years of experience in healthcare, care coordination, or a related field, preferably in a clinical or community healthcare setting. Experience in working with diverse patient populations and addressing complex healthcare needs.

Bachelor's (Required)
