Role Overview As a Hospital DME Liaison , you will serve as the primary point of coordination for patients being discharged with durable medical equipment (DME) and home oxygen. You will work closely with hospital staff, care coordination teams, and external partners to ensure timely delivery of equipment and a seamless discharge process. Key Responsibilities Serve as the main point of contact for hospital staff coordinating DME and home oxygen at discharge Collaborate with Care Coordination and hospital teams to facilitate timely DME delivery prior to discharge Utilize Cerner, HC+, and other EMR systems to manage and track patient orders Coordinate with external DME vendors to ensure proper documentation and delivery of equipment Partner with Customer Service teams to support timely order fulfillment and reduce delays Support seamless patient discharge workflows to help reduce readmissions Manage inventory for hospital DME supply areas Maintain clear communication between internal teams, vendors, and patients Perform other duties as assigned Qualifications Education•Required High School Diploma or GED Education•Preferred Associate's degree (in lieu of required experience)Experience Required:
