Full-Time About Walkers Group Home Walkers Group Home provides Tailored Care Management (TCM) services to individuals with behavioral health needs, ensuring they receive comprehensive, person-centered support. Our team is dedicated to improving the quality of life for our clients by coordinating care across medical, behavioral health, and social support systems. Position Overview We are seeking a Licensed Therapist (LCSW, LCMHC, LMFT, or equivalent) to join our team as a Tailored Care Manager . In this role, you will provide clinical support, care coordination, and advocacy for individuals receiving TCM services. You will work closely with clients, their families, and interdisciplinary teams to develop and implement individualized care plans. Key Responsibilities Conduct comprehensive assessments to determine clients' behavioral health and social support needs Develop and implement individualized care plans in collaboration with clients and interdisciplinary teams Provide individual and group therapy as part of an integrated care approach Coordinate medical, behavioral health, and social services to ensure continuity of care Monitor client progress, update treatment plans, and document all services in compliance with regulatory requirements Provide crisis intervention and support as needed Collaborate with community resources and service providers to enhance client outcomes Qualifications Master's degree in Social Work, Counseling, Psychology, or a related field Active NC licensure (LCSW, LCMHC, LMFT, or equivalent) Experience in care management, behavioral health, or community-based services preferred Knowledge of Tailored Care Management (TCM) guidelines and Medicaid-funded services is a plus Strong clinical assessment, documentation, and communication skills Ability to work independently and collaboratively in a team environment Benefits Competitive salary based on experience Flexible scheduling options (Hybrid/On-Site) Professional development opportunities Supportive team environment How to
