Job Description
The Peer Care Coordinator is a dynamic, dual-role position that blends the lived experience and relational power of peer support with the structured planning and advocacy of care coordination. This individual provides both emotional support and practical navigation through systems such as healthcare, housing, and employment for individuals impacted by substance use, incarceration, and behavioral health challenges. Working alongside care teams, probation/parole officers, and community organizations, the Peer Care Coordinator helps individuals set recovery-focused goals and access services needed for long-term stability. With personal experience in recovery or behavioral health challenges, this role offers both credibility and connection, fostering hope, resilience, and accountability in others. This position is ideal for someone who is both empathetic and organized - someone who can build trust through shared experience while also developing and managing individuals
Role Responsibilities:
- Serve as the primary point of contact and advocate for participants
- Share lived experience as a source of inspiration and empowerment for participants on their journey
- Assess participant needs and co-create care plans that promote recovery, stability, and independence
- Support participants in identifying recovery goals and monitoring progress; adjust care plans as needed
- Model healthy coping strategies, resilience, and community engagement
- Educate and connect participants to relevant community resources, services, and peer recovery supports
- Facilitate communication between participants and service providers to ensure seamless care delivery
- Maintain accurate records and ensure documentation complies with HIPAA and F5 Project program guidelines
- Participate in required training for both peer support and care coordination
- Attend team meetings, supervision, and professional development opportunities
- Travel up to 20% as needed to support program delivery and community outreach -Perform other duties as assigned to support
Required Qualifications:
- Certified peer support specialist training (or enrolled in an approved training program within 90 days of start date and certified within 45 days of completion)
- Lived experience with recovery from substance use, mental health challenges, justice involvement, or as a family member of someone who has
- High school diploma or GED required
- At least 1 year of relevant experience in direct service, social work, behavioral health, or case management (or 3 years experience with special populations)
- Valid driver's license and ability to travel across the region.
- Must be able to pass a background check.
- Strong communication, organizational, and interpersonal skills
- Comfortable working in dynamic environments, including correctional or justice-related settings
Preferred Qualifications:
- Experience in case management, parole/probation, reentry, or addiction services
- Peer Support certification or equivalent experience.
- Knowledge of Minnesota's behavioral health, housing, or corrections systems.
- Familiarity with rural program development, culturally responsive care, and tribal relationships
