Responsibilities Provide psychosocial support services to facilitate patient and family adjustment to illness. Social worker shall be knowledgeable about the psychosocial impact of disease on individuals and families as it relates to their age, stage of development, culture, physical and psychosocial competence. Capabilities include individual and group counseling, case management and advocacy services with the health care team. Works effectively with the integrated oncology program and multidisciplinary team to meet the psychosocial needs of the patient/family Gathers and assesses information regarding the patient's physical needs, mental status, family support system, financial resources, and other barriers to care Identifies and manages resource allocation and support services to meet the psychosocial needs of the patient/family and provides appropriate documentation of patient interactions and assistance provided. Develops a care plan with patient and family based on mutually agreed upon goals to enhance, maintain and promote optimal psychosocial functioning through cancer treatment and survivorship Provides individual counseling and crisis intervention and facilitates support groups as needed Attends Cancer Committee and Cancer Conferences when schedule permits Collaborate with other community resources and agencies and keep up to date on local and national resources Assists other staff members in the development and implementation of additional support services in response to patient needs Qualifications Masters degree in Social Work from an accredited graduate school of Social Work Clinical Social Work License is required. If not present at time of hire, must be obtained within one year Oncology experience preferred Knowledge of oncology disease and treatment process and effect of illness on patient population Knowledge of insurance, state and federal regulations specific to oncology Knowledge of oncology resources Knowledge of Social Work Theory and understanding and acceptance of Social Work Code of Ethics Knowledge of Crisis Intervention Theory Knowledge of community resources, state and federal programs Knowledge of individual, group and family therapy Highly skilled in written and verbal communications, interpersonal and public relations Skilled in organizational effectiveness, critical and analytical problem solving Ability to relate to people Ability to consistently present clear professional identity and purpose Ability to work with minimal supervision if necessary The compensation for this role includes a base pay range of $58,000-87,000, with the actual pay determined by factors such as skills, experience, education, certifications, geographic location, and internal equity. Additional compensation may be available through shift differentials, bonuses, and other incentives. Base pay is only a portion of the total rewards package.
Qualifications:
