With Aviator, you will work with Medicare patients throughout their healthcare journey, helping them overcome barriers to care and access the resources they need to thrive. We're looking for a mission-driven professional with a background in social work, community health work, or case management who excels at building trusted relationships with patients and community partners. This role is purpose-built for someone who has spent time in the field connecting people to resources, navigating complex systems, and advocating for those who need it most — and who is ready to do that work remotely with a dedicated caseload of Medicare patients. As a Health Advocate, you'll guide patients through their care journey, address barriers to care, and connect them to community resources. You'll build trusted relationships virtually, coordinate care with providers, conduct regular follow-ups, and ensure patients receive warm hand-offs to the services they need. You'll also help strengthen Aviator's community presence by fostering connections with local organizations and sharing insights on resource availability. This role is fully remote. What You'll Do Learn Aviator's systems, tools, technology, and partner network while bringing your own professional expertise to every patient interaction. Build strong, trusting relationships with older adults, where listening and empathy are the foundation for every interaction. Identify and prioritize Medicare patients' needs and assist them in maintaining a streamlined care continuum. Develop comprehensive patient care plans that holistically address social determinants of health, including food resources, transportation access, and support at home. Stay current on local resources that address social determinants of health (SDOH) and ensure patients are connected to the right services. Support patients through warm hand-offs, weekly follow-ups, and hands-on assistance — including navigating healthcare systems, applying for benefits, arranging transportation, accessing financial assistance, and connecting with caregiver resources.
What You Bring Required:
A background in social work, community health work, case management, or a closely related field. We're looking for candidates who have worked directly with individuals navigating health, social service, or benefit systems — whether in a formal clinical setting, a community-based organization, or a care coordination role. 2+ years of experience in care coordination, patient advocacy, community health work, social work, or healthcare navigation. Demonstrated knowledge of social determinants of health and experience working with diverse, underserved, or older adult populations. Tech-savvy and comfortable using digital platforms, electronic health records, secure messaging, and virtual meeting tools. Strong communication skills — able to support patients with empathy and collaborate effectively with healthcare providers and community organizations. Ability to manage a caseload, document clearly, and communicate proactively. Access to a reliable computer, high-speed internet, and phone to perform remote work effectively. Bilingual skills a plus, but not required. Additional Information 32-hour work week
