Job Description

Central Minnesota Council on Aging is committed to supporting older adults' by developing and coordinating community care, reducing isolation, and improving access to services. We invite you to discover how you can be a valuable part of our team, working to enhance the well-being of our aging community. At this time, Central Minnesota Council on Aging is seeking a full-time Minnesota Aging Pathways Resource Coordinator , 40 hours a week within a Monday-Friday, 8:00 am
  • 4:30 pm schedule.
In this role you will work directly with older adults and their families, providing person-centered assessments, options counseling, and care coordination to support successful community living and care transitions. Key Responsibilities Complete in-person and phone-based, person-centered assessments for older adults and caregivers. Provide options counseling related to long-term care services, benefits, housing, caregiver support, and financial resources. Assist individuals and families in navigating next steps following hospital, rehabilitation, or nursing facility stays. Coordinate services and referrals with healthcare providers, community partners, and service agencies. Advocate on behalf of clients, when needed, to support access to appropriate services. Manage an ongoing caseload and document assessments, plans, and services accurately. Qualifications Bachelor's degree in social work, gerontology, nursing, or a related human service field. Preferably 2 or more years of professional experience in case management, discharge planning, care coordination, long-term care, health care, or related social services settings. Knowledge of aging services, community resources, and long-term supports. Strong communication, organizational, and follow-through skills. Location & Work Arrangement The position is based in St. Cloud, MN. Employees living within 45 miles of the office are eligible to follow a hybrid schedule with two days per week working on site. Employees residing within our 14-county service region (Benton, Cass, Chisago, Crow Wing, Isanti, Kanabec, Mille Lacs, Morrison, Pine, Sherburne, Stearns, Todd, Wadena, and Wright counties), but more than 45 miles from the office are eligible for a fully remote schedule, with in-person client work.
