Job Description

Job Summary The Care Coordinator plays a vital role in ensuring that patients receive comprehensive and efficient care throughout their skilled rehabilitation journey with our center. This position involves collaborating with insurance companies, hospitals, healthcare providers, patients, and their families to facilitate communication, obtain eligibility and authorizations, and coordinate case management for the center. The ideal candidate will possess a strong understanding of managed care, insurance authorization, and skilled documentation for nursing and therapy services. Duties Serve as the primary point of contact for patients, addressing inquiries and concerns related to their skilled care stay. Determine financial and clinical eligibility. Obtain insurance authorizations to facilitate patient admission. Deliver NOMNC and ABN notifications. Maintain professional communication with insurance representatives, patients, peers. Coordinate patient appointments, ensuring timely access to services and follow-up care. Set meetings with patient responsible parties to coordinate plan of care and set discharge goals. Collaborate with healthcare providers to streamline communication and enhance patient care. Educate patients on treatment options, procedures, and necessary follow-up actions. Ensure compliance with healthcare regulations and maintain confidentiality of patient information. Coordinate service for discharge planning including DME, home health, and other support services. Qualifications Proficiency in medical terminology is essential for effective communication within the healthcare environment. Strong understanding of Medicare Part A and Managed Care / Medicare Advantage as it relates to services in a skilled nursing environment. Previous experience in a skilled nursing facility is highly preferred with experience in Case Management, LPN or RN, Social Services, Admissions, Therapy, or similar roles. Strong organizational skills with the ability to manage multiple tasks simultaneously while maintaining attention to detail. Excellent interpersonal skills to foster positive relationships with patients, families, and healthcare professionals. Ability to work collaboratively within a team-oriented, fast-paced environment while demonstrating initiative in problem-solving. Must maintain excellent attendance. This Care Coordinator position offers an opportunity to make a meaningful impact on patient care while working in a dynamic healthcare setting. This is not a remote or work from home position.
