Job Description
Experteer Overview In this remote IRIS Consultant role, you help participants navigate Wisconsin's IRIS program to meet long-term care goals. You build trusting relationships, assess needs, and connect people with community resources while developing customized IRIS plans. You work closely with a team to support plan implementation, budget stewardship, and compliance, including HIPAA. The role offers meaningful impact through person-centered planning and collaboration across settings, with strong work‑life balance and team support.
Compensation / Benefits
Meet IRIS participants in person at least four times per year with one annual home visit
Provide program orientation and explain participant rights, responsibilities, and budget use
Assess goals, community connections, employment interests, and Self-Directed Personal Care options
Develop and monitor participant outcomes within IRIS budget and DHS requirements
Document orientation, planning, progress, and modifications in the WISITs data system within 48 business hours
Research and share suitable community resources and natural supports
Assist with budget interpretation and provider billing processes via liaison with Fiscal Employer Agency
Build engaging, confidential relationships and communicate program changes effectively
Collaborate with IRIS Consultant Agency staff to implement participant plans
Attend monthly in-person team meetings and participate in regular check-ins
Perform additional duties as assigned Tasks
At least 2 years in health care (preferably care coordination)
At least 1 year serving IRIS target groups or equivalent experience
Bachelor's degree in social work, psychology, human services, counseling, nursing, special education, or related field (or 4 years of commensurate experience)
Valid driver's license, reliable transportation, and adequate auto insurance for job-related travel
Ability to work independently and build professional relationships
Knowledge of long-term care programs and self-determination principles
Strong problem-solving, time-management, and multi-project prioritization
Excellent verbal and written communication; proficient in Microsoft Office
Comfort working with diverse populations and varying settings
HIPAA confidential handling and teamwork skills Key requirements
