Key Responsibilities The Triage Coordinator is responsible for the coordination of all after hours calls, after hours orders, and after-hours communication with on-call provider, patients, facilities, and families for Hospice. Responsibilities are completed with limited supervision and after hours. This position requires initiative, creative problem resolution, time management, excellent phone and electronic communication skills, service excellence and a desire to work within a practice that centers around and with a geriatric population. In this role you will be responsible for: Communicates significant findings, problems, changes in condition or environment to the primary care providers and other personnel involved with patient care Performs well under stress and in emergency situations Assumes primary responsibility for "after hours" including planning, coordinating, implementing, and assisting with the evaluation of the plan of care for patients, families and facilities Develops and maintains positive working relationships with others. Helps others when needed without necessarily being asked to do so Maintains confidentiality of patient and employee information. Shares information only in accordance with policy Answer telephone calls after hours and responds to any concern or change in patient condition Provides support and direction to agency staff, health care professionals patients, and families related to health care concerns Required Qualifications High School Diploma or GED Minimum one (1) year of experience in medical field, medical office experience or related position Ability to operate computer equipment, Fax and photocopy machines necessary Proficient in Microsoft Office Suite, including Outlook, Word, Excel, and PowerPoint Knowledge of medical terminology Excellent verbal and written communication skills Desired Qualifications Associate degree preferred CNA or MA certification preferred Experience with geriatric population and/or caregivers preferred Familiarity with HCHB and Ethizo EMR preferred On-Site Office Locations Bingham Farms, Grand Rapids, Jackson, Kalamazoo, and Saginaw, Michigan; and Maumee, Ohio Hours This position offers a flexible schedule with a combination of on-site and remote work.
