Job Summary As a Veteran Directed Care (VDC) and Elderly Services Program (ESP) Care Coordinator, you serve as a trusted partner to older adults, caregivers, and veterans. You'll assess needs, develop personalized care plans, monitor services and budgets, and connect clients with essential community resources—supporting safe, independent living at home. What You'll Do Veteran Directed Care Coordination Serve as the primary contact for all VDC referrals Conduct initial screenings, in-home assessments, and enrollment support Guide veterans through spending plans and budget development Monitor monthly budgets to ensure compliance and address concerns Maintain ongoing in-home visits and client support Submit required documentation and reports to VA partners Participate in quarterly VA meetings Elderly Services Program Coordination Conduct home visits to assess eligibility, needs, and risks Develop and manage individualized care plans Maintain a client caseload and provide ongoing support through visits and calls Coordinate services using cost-effective approaches while meeting client needs Educate clients and caregivers on available services and systems Support discharge planning and transitions Documentation & Compliance Maintain accurate client records and required documentation Ensure all services are properly authorized and supported Adhere to confidentiality policies and program standards Cost Share & Financial Coordination Complete income verification and cost-share documentation Monitor copay status and communicate with clients and internal teams Educate clients on financial responsibilities Agency Impact Contribute to COA goals, programs, and improvement initiatives Participate in committees, training, and community engagement efforts Qualifications Bachelor's degree in Nursing, Social Work, Gerontology, Psychology, OR At least one year of experience in home health care, case management, geriatrics, or related field Knowledge of issues affecting older adults and community resources Strong interpersonal, communication, and customer service skills Excellent organization and ability to manage multiple priorities independently Ability to adapt in a fast-changing environment Strong written documentation skills Proficiency in Microsoft Office and data systems Reliable transportation and valid driver's license COA-approved high-speed internet for remote work What Success Looks Like Veterans confidently managing their care and budgets Older adults safely aging in place with coordinated support Strong relationships built on trust, empathy, and advocacy You feeling empowered, supported, and fulfilled in your work Ready to Make a Meaningful Difference?
