Job Description
Overview of the
Position:
The Value Based Care Coordinator (VBCC) will implement strategies to optimize practice performance in HopeHealth value-based care arrangements. The VBCC will be responsible for activities that help practices improve the health status and care for members attributed to HopeHealth s Medicare Advantage, Medicaid, and Commercial health plans. The key function of the role is to coordinate with the Quality team to implement strategies and workflows that will drive quality and performance improvement. Qualifications (Required) Four-year degree in business or healthcare related field. Working knowledge of Microsoft Office applications. Knowledge of data reporting. Qualifications (Preferred) Prior experience with care coordination/case management in a medical setting. Experience working with electronic medical records. Skills and Abilities Demonstrated understanding of value-based care principles, especially as they relate to population health management and improvement in health care quality. Solution oriented team member who can facilitate quality improvement initiatives. Experience with quality and process improvement methodologies and measures. Interpersonal skills and ability to demonstrate cooperation and teamwork. Accountable, self-starter with exceptional organizational and time management skills. Excellent verbal and written communication skills. Position Responsibilities The Value Based Care Coordinator is responsible for assisting practices improve their overall performance on quality benchmarks in value-based arrangements through activities such as identification, outreach, coordinating patient visits for preventative care, conducting chart reviews, and assisting with data submission to payers.Physical Requirements:
Must be able to lift 30 pounds. Vision and hearing corrected to within normal limits is required. Must have manual dexterity to key in data; utilize computer, grab, grip, hold, tear, cut, sort, and reach.Similar remote jobs
