Resident Assessment Coordinator

We are seeking a highly skilled and experienced Resident Assessment Coordinator (RAC) to join our team at Dickson Health and Rehab in Dickson, TN. As an RAC, you will play a crucial role in ensuring the accuracy and timeliness of resident assessments, care planning, and reimbursement optimization, while supporting quality resident outcomes in a long-term care setting. This is a full-time position with a salary range of $30-$35 per hour.

Key Responsibilities:

Complete and submit accurate and timely Minimum Data Set (MDS) assessments (admission, quarterly, annual, significant change, discharge) Coordinate interdisciplinary care plan meetings Ensure comprehensive, individualized care plans are developed and updated Maintain compliance with RAI Manual guidelines Ensure compliance with CMS, Medicare, Medicaid, and state regulations Maintain survey readiness related to MDS accuracy and documentation Optimize reimbursement through accurate coding and documentation Monitor PDPM components and case mix index (CMI) Collaborate with therapy, nursing, and billing teams

Qualifications:

Current RN license in Tennessee MDS experience in long-term care preferred Strong knowledge of RAI Manual and PDPM Experience with electronic medical records (EMR) systems Strong organizational and time management skills

Preferred Qualifications:

MDS Certification (RAC-CT or similar) Experience with Medicare/Medicaid reimbursement systems Prior experience in skilled nursing facility (SNF) setting
