Full Job Description Job Description:
The Post-Acute Benefits & Resource Coordinator serves as a specialized resource within the Community Care Network and Case Management department focused on assisting uninsured, underinsured, Medicaid-pending, and financially complex patients requiring post-acute care services. This role collaborates with patients/families, hospital case management, payers, financial counseling, and post-acute providers to support payer source identification, insurance navigation, financial resource coordination, and timely access to appropriate post-acute care services. The position supports complex discharge planning and reduction of avoidable inpatient days associated with financial or payer-related barriers to post-acute placement. Minimum Qualifications Bachelor's Degree in Social Work, Healthcare Administration, Human Services, or related healthcare field preferred. Relevant healthcare, post-acute care, Medicaid/TennCare, case management, financial counseling, or skilled nursing facility experience may be considered in lieu of degree requirements. Minimum of two (2) years experience in healthcare, case management, social work, skilled nursing facility, post-acute care, insurance navigation, or related field preferred. Preferred Qualifications Skilled Nursing Facility (SNF) or post-acute care experience strongly preferred. Experience with Medicaid/TennCare processes and post-acute placement barriers preferred. Knowledge of insurance authorization and payer processes preferred. Experience collaborating with community agencies and post-acute providers preferred.
