Job Description

Job Overview:
We are seeking a dynamic and detail-oriented Medical Healthcare Coordinator to join our healthcare team. In this vital role, The Healthcare Coordinator is responsible for supporting the health and medical needs of supported persons served by SOAR TN through coordination of medical services, medication oversight, documentation compliance, and communication with healthcare providers. This position plays a critical role in ensuring all medical documentation, medication administration processes, and healthcare coordination activities remain compliant with DIDD, TennCare, and organizational standards. The Healthcare Coordinator works collaboratively with the Medical Department, House Managers, Program staff, pharmacies, and external healthcare providers to ensure continuity of care and timely response to medical needs. Responsibilities Count medications and ensure all MAR (Medication Administration Record) forms are accurate and current. Conduct weekly audits of MAR books in each house to ensure all transcription and documentation requirements remain compliant with DIDD and state regulations. Coordinate and order medication refills in a timely manner to ensure uninterrupted medication availability for supported persons. Verify that medication side effects are documented appropriately on required forms. Ensure all physician orders are properly maintained within MAR books for each medication prescribed. Monitor controlled medications to ensure proper counting procedures, documentation, and double-lock storage compliance. Maintain and organize medical paperwork and records within Medical Comprehensive Books. Work collaboratively with House Managers to ensure Medication Certified staff maintain proper medication training and documentation standards. Ensure medication variances are documented, reported, and submitted according to agency policy and compliance standards. Coordinate medical appointments, treatment plans, and follow-up care as directed. Communicate with healthcare providers, pharmacies, guardians, and interdisciplinary teams regarding healthcare coordination needs. Assist with healthcare education and care instruction communication as directed by medical professionals. Maintain confidentiality of medical and protected health information in accordance with HIPAA standards. Demonstrate professionalism and teamwork in all interactions with employees, supported persons, families, healthcare providers, and external agencies. Perform additional medical department duties and special projects as assigned. Qualifications and Abilities High school diploma or equivalent required; additional education in healthcare, medication management, nursing support, or related field preferred. Minimum of two years of experience as a DSP, House Manager, Program Coordinator, or related healthcare support role preferred. Valid driver's license with acceptable driving record required. Ability to obtain and maintain CPR, First Aid, and Medication Administration certification. Ability to pass all required background screenings and credentialing requirements. Ability to effectively read, write, and communicate verbally in English. Basic computer and documentation skills required for healthcare tracking and communication. Strong organizational, problem-solving, and time management abilities. Ability to maintain confidentiality and professionalism when handling sensitive medical information. Ability to work flexible hours as needed, including evenings, weekends, or holidays. Must meet all licensing, regulatory, and credentialing agency requirements. Core Competencies Attention to Detail
