Client Care Consultant - Full-Time Abilene, TX - Local travel required

As our Client Care Consultant, you'll be the heartbeat of that experience — conducting care consultations, building individualized service plans, and serving as a trusted point of contact for clients, families, and referral partners. PRIMARY RESPONSIBILITIES
- Conduct care consultations with prospective clients and family members
- Develop and maintain individualized service plans
- Coordinate client and Care Professional introductions
- Conduct quality assurance visits and follow-up communication
- Maintain accurate documentation and client records
- Build and maintain positive relationships with clients, families, and referral sources
- Identify opportunities to improve client satisfaction and care continuity
- Collaborate with scheduling, recruiting, and leadership teams
- Support problem resolution and service recovery when concerns arise
- Participate in on-call support and community outreach activities as needed
QUALIFICATIONS
Required
- 1+ year in home care, healthcare, senior services, or case management
- Strong communication and interpersonal skills
- Calm and professional under pressure
- Strong organization and time management
- Valid driver's license and reliable transportation
- Proficiency with Microsoft Office Preferred
- College degree
- Senior care or healthcare coordination experience
- Consultative or relationship-based sales experience Schedule
- Primarily Monday-Friday office hours
- Flexibility for occasional evenings and weekends
- On-call rotation may be required
