This role combines hands-on case management with supervisory responsibilities, ensuring seamless coordination of services across hospital, hospice, behavioral health, and acute care environments. The ideal candidate will possess a strong background in critical care, hospital experience, and medical terminology, enabling them to effectively manage complex cases and guide multidisciplinary teams. Responsibilities Oversee and coordinate patient care plans, discharge planning, and intake processes to ensure timely and appropriate services. Supervise field staff and case managers, providing guidance on clinical protocols and ensuring compliance with healthcare regulations such as HIPAA. Conduct utilization review and utilization management to optimize resource use while maintaining high-quality patient care. Collaborate with hospital teams, social workers, physicians, and other healthcare professionals to facilitate seamless communication across departments. Manage documentation within EMR (Electronic Medical Record) and EHR (Electronic Health Record) systems, ensuring accuracy in medical coding (ICD-10, ICD-9, CPT) and compliance with HIPAA standards. Lead efforts in discharge planning, behavioral health coordination, and managed care initiatives for diverse patient populations including pediatrics and trauma center cases. Participate in quality assurance activities related to ICU experience, Level I/II trauma centers, and specialty areas such as PICU or emergency medicine. Skills Extensive critical care experience with a strong understanding of hospital medicine, ICU protocols, and emergency medicine procedures. Proficiency in medical terminology, ICD coding (ICD-10/ICD-9), CPT coding, and utilization review processes. Experience working within hospital settings such as Level I or Level II trauma centers; PICU or OR experience is highly desirable. Knowledge of social work principles, behavioral health management, discharge planning, and case management strategies. Familiarity with EMR/EHR systems and healthcare compliance standards including HIPAA regulations. Strong organizational skills with the ability to manage multiple cases efficiently while maintaining attention to detail. Excellent communication skills for effective collaboration with multidisciplinary teams across hospital environments.
