Experience Position Summary:
Primarily responsible to Care Coordinator Supervisor for Care Coordinator duties/
Qualifications:
 Master's or Batchlor's Degree in health and human services field, or high school diploma and minimum two years' experience working with person who have disabilities or two years of specialized training in the field of health and human services. Valid Texas driver's license with insurable driving record. Essential Duties & responsibilities: Responsible for coordinating all non-waiver elements of the Person Directed Plan of individuals in the CALAB, Inc. HCS programs. Responsible for Compliance with all the principles and policies of the CALAB, Inc. HCS Program. Responsible to assure that appropriate contractor are assigned to the individuals served, and that all elements of an individual's needs are addressed by the appropriate professional staff or contractor, and that all professional appointments are made in a timely manner. Responsible for conducting individual plan of care meetings and attendance at other meetings as requested by the consumer. Responsible for informing consumers and families progress towards meeting their goal. Responsible for appropriate correspondence with family members and/or responsible parties as necessary for delivery of the implementation Plan. Responsible for assuring that Implementation Plans are developed in accordance with the person Directed Plan. Responsible to ensure that proper documentation is maintained in the individual file to justify utilization of fuds paid for services to the individuals. Responsible to ensure that all HCS documents are completed, entered, and given to the appropriate entity within the timeframes, ie: MR/RCs, IPCs, ICAPs, physical site reviews, etc... Responsible for training direct service staff and monitor training. Responsible for monitoring progress to training, making necessary changes to the Implementation Plan or IPC, and completing quarterly reviews. Responsible for ensuring ancillary services are completed in accordance with the PDP and Implementation Plan, including development of he Behavior Therapy Program with guidance form he Psychologist. Monitor progress of the Behavior Therapy Program. Complete and submit packets for a Level of Need increase and bump up. Responsible for providing appropriate repots and seeking guidance from CALAB, Inc. HCS Advisory committee. Responsible for other duties as assigned by the Regional Program Director.

Physical and Sensory Skills:
Ability to communicate with and assist all consumers. Ability to read, write, and converse in English and in a professional manner.

HCS Program, understand that I am responsible for, at a minimum, the following tasks: Development of the Implementation Plan in accordance with the Person Directed Plan and Individual Plan of Care. Coordinating the delivery of each person's Individual Plan of Care. Coordinating and monitoring the delivery of waiver and generic services. Integrating various aspects of service delivered under the waiver and through other sources. Ensuring documentation is completed in a timely manner, ie: MR/RC, IPC's , ICAP, etc... I have read the above description and agree to assure, to the best of my abilities, that i can provide these areas of assistance to the individuals I serve in the CALAB, Inc. HCS Program. If at any time during my tenure as an employee I have questions or need additional training in any area of assistance to the individuals, I will request this form my supervisor.
