Full-Time, Monday - Friday, 8:00 AM - 5:00 PM About This Opportunity Join the compassionate team at Lone Star Family Health Center as a Behavioral Health Care Coordinator. In this meaningful role, you'll be the vital connection between patients and our behavioral health services, ensuring every individual receives the support and care they deserve. You'll coordinate all aspects of our behavioral health program while advocating for underserved populations with empathy and professionalism. What You'll Do Serve as the welcoming first point of contact, greeting patients and visitors with warmth and professionalism while routing them appropriately Coordinate all aspects of the Behavioral Health program, including patient intake, information updates, co-pay collection, and appointment scheduling Collaborate seamlessly with the Behavioral Health team to coordinate consultations and implement treatment recommendations as directed by providers Schedule patient appointments according to established provider and department-specific guidelines Triage behavioral health emergencies and concerns in partnership with therapists, ensuring appropriate and timely responses Proactively follow up with patients who have missed appointments or fallen behind on treatment Support patients in completing their treatment plans and achieving positive health outcomes Serve as an advocate for underserved individuals, demonstrating sensitivity to each client's unique cultural background, circumstances, and needs
What We're Looking For Bilingual Proficiency:
Fluent in English and Spanish (required)
Healthcare Experience:
Minimum 2 years of work experience in the healthcare industry with significant direct patient care involvement
Education:
High School diploma or General Education Degree (GED)
Communication Skills:
Excellent verbal and written communication abilities with the ability to interact professionally with diverse populations
Organizational Skills:
Strong ability to manage multiple tasks, prioritize effectively, and maintain accurate patient records
Compassion and Empathy:
Genuine commitment to serving vulnerable and underserved populations with respect and cultural sensitivity Nice-to-Have Skills Graduation from a Medical Assistant program Previous experience in a behavioral health or mental health setting Certification as a Medical Assistant (CMA) or similar healthcare credential Experience with electronic health records (EHR) systems About Lone Star Family Health Center Lone Star Family Health Center is dedicated to providing comprehensive, compassionate healthcare to our community, with a special focus on serving underserved and vulnerable populations.
