ESSENTIAL DUTIES
Models a service and solutions oriented attitude towards others, takes ownership in solving problems and takes the initiative to make things better, is friendly and courteous to residents, patients and co-workers Ensure resident/patient, visitor, and co-worker safety by remaining aware of and reporting any new or existing safety hazards Maintain good attendance record Recruit, hire, train, coach, and evaluate departmental staff (management staff only) Assess residents and completes MDS according to facility policy Develop and revise resident service plans to reflect residents current status Monitor residents for changes in condition, ensure MD, and family, a are notified and appropriate documentation is reflected in the medical record Assess residents initially and annually and complete resident service plans on an annual or as needed, basis Chair the Interdisciplinary Service Plan Meeting Maintains the Resident's Roster and Acuity Levels within the department (i.e., State Boards, QA monitor) Review and follow-up all consultant reports in a timely manner Reviews monthly physician's orders and ensures accuracy Monitors psychotropic behavior sheets and makes recommendations as needed Complete performance evaluations of all licensed nurses, mediation aides and CNA assigned to AL Notify Director of Assisted Living Services of incident reports concerning abuse Thoroughly investigate any complaints and concerns and report to Director of Assisted Living Services Attend all assigned committee meetings Review incident report documentation and ensures follow-up according to facility policy Make rounds to verify clinical compliance and quality of care and provide coaching and disciplinary action as indicated within the facilities policies and procedures Ensures all Life Enrichment programs/QA monitors are implemented on assigned units Perform change nurse duties, in cases of staffing emergencies, as required to meet the resident's immediate needs Other duties as assigned, including filling in as Nurse duties required, as needed
MINIMUM QUALIFICATIONS
Valid Texas LVN license Demonstrated supervisory and managerial skills Knowledge of current professional nursing theory and practice Excellent technical, assessment and documented skills Ability to organize workflow, respond to emergencies and note detail Ability to maintain a positive, stable disposition in performing work in association with staff and residents Ability to maintain a high level of performance in an environment of frequent demands and changes Ability to read, write and effectively communicate in English Excellent verbal and written communication skills Ability to perform Essential Duties Genuine caring for and interest in the care of mature adults, persons with handicap and hospice patients Ability to perform work tasks within the Physical Demand Requirements Ability to comply with the Resident Bill of Rights and Employee Responsibilities Ability to comply with organizational and departmental safety policies and procedures
