Role:
W. Leo Daniels Towers is a 99-unit independent living community serving elderly residents and persons with disabilities. As the Service Coordinator, you are the on-site advocate who helps residents access the services and resources they need to continue living independently — healthcare, benefits, meal programs, transportation, and social connections. This position is funded through HUD's Service Coordinators in Multifamily Housing (SCMF) program and is dedicated entirely to resident well-being. This role requires a regular on-site presence with flexibility for some administrative work to be completed offsite.
What You'll Do:
Resident engagement & assessment: Build relationships through frequent interactions. Conduct annual assessments, develop individualized service plans, and maintain a property-wide profile of resident needs. Resource connection & referrals: Build and maintain a local provider directory. Refer residents to healthcare, food assistance, transportation, mental health, and benefits programs (Medicaid, SNAP, SSI, Medicare Part D). Follow up to ensure services are received. Advocacy & education: Advocate for residents with service providers and government agencies. Organize on-site wellness events and educational programming. Help residents build informal support networks. Documentation & compliance: Track all interactions, referrals, and outcomes in FamilyMetrics or AASC Online. Maintain confidential case files. Support preparation of HUD-required reports (Standards for Success, HUD-50080-SCMF). Participate in QA reviews and HUD monitoring visits.
Staff coordination:
Educate property management staff on aging-in-place issues. Meet regularly with site staff to share information while maintaining role separation. This position is funded by a HUD grant and is dedicated exclusively to resident supportive services. The Service Coordinator does not perform property management functions (rent collection, lease enforcement, maintenance, or administrative tasks for the management company). This separation is required by HUD.

Requirements:
Requirements Bachelor's degree in social work, psychology, counseling, or related field preferred; equivalent work experience accepted 2-3 years providing social services, case management, or community-based support to elderly, disabled, or family populations Working knowledge of social services and community resources in the Houston area Ability to complete 36 hours of HUD-required training within 12 months of hire and 12 hours of continuing education annually Reliable transportation; bilingual (English/Spanish) a plus
