Full Job Description Job Purpose or Summary:
The Care Coordinator (CC) is a key point of contact for clients primarily responsible for ensuring the quality of ongoing care. The CC will oversee all care plans, ensure proper oversight and represent Heroes Home Care with clients and stakeholders as well as referral partners in the community.
Essential Duties and responsibilities:
(1-4 - Will discuss in more detail) 1. Client Care 2. Network Marketing 3. Operations 4. Perform other duties as assigned
Knowledge and Skill Requirements:
Two years' experience in Geriatrics and/or Care Management Hands-on Caregiving Experience with Seniors is required Previous Scheduling experience preferred CNA License preferred Ability to interface effectively and professionally with client committee, including family, physicians, attorneys, etc. Ability to work in a team environment Ability to lead and supervise caregivers, directing them in providing the highest-level care to meet client needs Excellent listening, written and verbal communication skills
