Job Description

Post Acute CoordinatorPosition Summary The Post Acute Coordinator is responsible for driving an exceptional patient and family experience while supporting census growth and smooth admission transitions. This role partners with patients, families, referral sources, and the interdisciplinary team to ensure concerns are addressed quickly, admissions are well-coordinated, and every new patient has a strong, welcoming start. This position plays a key role in protecting the reputation of the facility through service excellence, responsiveness, and accountability.
Essential Duties:
Patient & Family Experience Serves as a primary point of contact for patients and families during admission and early stay. Rounds routinely to ensure patient and family needs are being met. Provides clear, consistent communication regarding care and services. Builds trust by being visible, approachable, and proactive. Identifies opportunities to improve the overall resident experience and follows through. Service Recovery & Concerns Responds immediately to patient and family concerns. Owns concerns through to resolution—does not pass them off. Partners with department leaders to resolve issues quickly and effectively. Documents concerns and trends to support ongoing improvement. Escalates concerns appropriately when needed. Census & Relationship Support Supports daily census goals by helping remove barriers to admission. Maintains awareness of referral activity and admission opportunities. Communicates effectively with internal and external partners to support timely admissions. Helps ensure a consistent, positive experience that encourages referrals and returns. Admissions Coordination Prepares for all incoming admissions to ensure readiness across departments. Confirms necessary documentation, insurance information, and clinical details are in place. Coordinates with nursing, therapy, and support services prior to admission arrival. Supports a smooth and welcoming admission experience for patients and families. Ensures rooms, equipment, and staff are prepared. Team Collaboration Acts as a liaison between patients, families, and the interdisciplinary team. Communicates challenges, needs, and opportunities to the appropriate team members. Participates in daily census or stand-up meetings as needed. Supports a culture of teamwork, accountability, and service. Qualifications Experience in skilled nursing, post-acute care, or healthcare preferred. Strong communication and customer service skills. Demonstrated ability to resolve concerns and build relationships. Organized, responsive, and able to manage multiple priorities. Working knowledge of admissions processes, payer sources, and post-acute workflows preferred. What Success Looks Like Patients and families feel heard, supported, and confident in their care. Concerns are addressed quickly with clear resolution. Admissions are organized, timely, and welcoming. The facility maintains strong census through effective coordination and experience. The team operates with a high level of accountability and collaboration.
