Monday-Friday, 8:00 AM - 5:00 PM Work setting: On-site — Dallas-Fort Worth, TX [Plano] with Hybrid-remote options About the role Ascent Healthcare Texas is hiring a Care Coordinator to be the welcoming first point of contact for people seeking outpatient behavioral health care in the Dallas-Fort Worth area. You'll guide patients from their first call through scheduling, intake, and ongoing coordination — making sure no one falls through the cracks. If you're organized, calm under pressure, and genuinely care about helping people access care, we'd love to meet you. What you'll do Answer incoming patient calls and referrals in a timely, professional, and trauma-informed way Gather initial information, conduct non-clinical safety screening, and escalate clinical concerns to the appropriate licensed staff Schedule, confirm, and reschedule assessments and appointments within established timeframes Manage intake and referral tracking so no inquiry or referral is ever lost or delayed Conduct follow-up outreach for missed calls, no-shows, and pending assessments Support daily census tracking and flag patterns of disengagement to the clinical team Track insurance authorizations — start/end dates, approved units, and upcoming expirations Assist with referrals, discharge planning, and connecting patients to follow-up care and community resources Keep accurate, up-to-date records across all tracking systems and the EHR What we're looking for High school diploma required; bachelor's in behavioral health, healthcare administration, or a related field preferred 1-3 years in healthcare, behavioral health, intake, scheduling, or coordination roles preferred Experience in mental health or substance use settings strongly preferred Strong organization and multitasking; comfortable managing high-volume workflows with attention to detail Excellent communication and interpersonal skills with a calm, trauma-informed approach Familiarity with EHR systems (e.g., AdvancedMD, Netsmart) and insurance authorization processes is a plus Why Ascent Healthcare Texas At Ascent Healthcare Texas, we build systems that lift people up.
