Job Description

Job Summary Our client is seeking a Census Management Specialist to play a critical role in maintaining accurate patient data and ensuring seamless communication with healthcare providers. This position supports the pharmacy's mission of delivering high-quality care by reconciling patient records, analyzing census trends, and collaborating across departments to align clinical data with billing requirements.
Key Responsibilities Census Management:
Monitor and maintain accurate patient census records; reconcile data with referrals, admissions, discharges, and transfers from healthcare facilities.
Coordination:
Serve as the primary point of contact for census-related inquiries; coordinate with internal pharmacy teams to ensure timely prescription processing.
Financial Alignment:
Work closely with billing and reimbursement teams to ensure census data aligns with specific payer and insurance requirements.
Data Analysis:
Generate and analyze daily, weekly, and monthly reports to track census trends, demographics, and service utilization for leadership review.
Problem Solving:
Identify discrepancies or patterns in data and recommend corrective actions; use reason and analytical skills to resolve issues.
Regulatory Compliance:
Ensure all census management practices comply with regulatory and accreditation standards; maintain meticulous documentation for auditing.
Process Improvement:
Identify opportunities to streamline workflows, improve data accuracy, and provide training or guidance to staff as needed.
Professional Communication:
Speak clearly and persuasively in all situations; listen effectively and provide high-quality customer service to clinical partners.
Teamwork:
Contribute to a positive team spirit by giving and welcoming feedback and supporting organizational goals.
Quality Assurance:
Demonstrate accuracy and thoroughness in all work to promote the highest quality of patient care. Qualifications High School Diploma or GED 4+ years of experience in pharmacy operations, process optimization, or healthcare coordination.
Pharmacy Knowledge:
Experience in pharmacy billing, authorizations, or pharmacy technician roles.
Analytical Skills:
Strong ability to gather and analyze information skillfully to develop alternative solutions.
Technical Proficiency:
Ability to build knowledge in pharmacy systems and maintain organized documentation.
Preferred:
Prior experience in Provider Relations or Quality Assurance within a pharmacy setting.
