Resident Care Coordinator Rosewood Assisted Care - 3.8 Salt Lake City, UT Job Details Full-time $18.00 - $18.50 an hour 1 day ago Qualifications Care facility emergency preparedness Staff scheduling Care facility maintenance management Care coordination CNA Long-term care assistance Full Job Description About the Role Rosewood Assisted Care is seeking a strong, hands-on Resident Care Coordinator (RCC) to oversee resident care, medication systems, and clinical operations within our community. This role is responsible for ensuring safe, compliant, and high-quality care for residents while providing leadership and oversight to med tech and caregiving staff. In addition to core RCC responsibilities, this position will also assist the Executive Director with administrative and operational tasks to support the overall function of the building. This is not a desk-only role—this position requires active involvement on the floor, strong attention to detail, and the ability to lead and hold staff accountable. Schedule Expectations Ideally work 2-3 shifts per week on the floor Approximately 16 hours per week of scheduled office/administrative time Office time is structured, task-focused Maintain a consistent and planned schedule Be available to assist with coverage or solutions when needed Key Responsibilities Care Coordination & Clinical Oversight Ensure all service plans are individualized, accurate, and up to date Coordinate with nurse to ensure care plans reflect current assessments Monitor and document changes of condition Communicate updates with nurse, Executive Director, and families Coordinate care with providers, hospice, and home health Maintain oversight of MAR accuracy and physician orders Medication Management Oversee all medication systems for accuracy, safety, and compliance Supervise Lead Med Techs and provide ongoing support and accountability Conduct routine med pass observations and audits Ensure medications are ordered, received, and documented correctly Maintain strict narcotic control, including routine counts and discrepancy follow-up Ensure new orders and hospital discharge medications are implemented promptly Staff Leadership & Accountability Provide direct oversight of Lead Med Techs and caregiving staff Hold staff accountable to med pass procedures and documentation standards Coach, train, and correct staff as needed Support floor operations and step in when necessary Administrative Support (Assistant to Executive Director) Assist with scheduling support and staffing coordination Help ensure coverage and assist in problem-solving staffing gaps Support onboarding and training of new staff Help maintain organization of clinical systems and records Assist with general administrative and operational needs of the building Quality Control & Compliance Perform routine audits (med carts, MARs, documentation) Ensure compliance with assisted living regulations Identify and correct issues proactively Assist with incident review and follow-up Maintain readiness for surveys and inspections Expectations Must have CNA Maintain a consistent and reliable presence in the building Be proactive—identify and resolve issues before they escalate Keep medication systems organized, accurate, and audit-ready Communicate clearly and consistently with staff and leadership Be willing and expected to assist with shift coverage when staffing gaps arise Remain responsive and available to address urgent issues as they occur, including outside of scheduled hours when necessary Why Rosewood Team-focused environment with leadership support Opportunity to grow into administrative and leadership roles Direct impact on quality of care and operations Apply Today If you are a proactive leader who values accountability, strong systems, and high-quality resident care, we encourage you to apply.
