Responsibilities Develop and manage individualized discharge plans in collaboration with medical staff, social workers, and families Conduct thorough assessments to determine each resident's post-discharge care needs Provide ongoing case management to support residents throughout their stay and transition Connect residents and families with community resources including home health, rehabilitation, mental health, and support services Coordinate with outpatient providers and post-acute care facilities to arrange follow-up services Maintain accurate documentation in compliance with HIPAA regulations Requirements Experience in discharge planning, case management, or a related healthcare role Strong assessment and communication skills Familiarity with community resources and post-acute care options Knowledge of medical terminology and HIPAA regulations Ability to work effectively within a multidisciplinary team If you are interested in joining the Copper Family please apply with your resume here or feel free to contact the building directly at 801-280-2273.
