Work Schedule:
M-F 8:30 a.m.- 5:00 p.m Join the Hampton-Newport News Community Services Board and play an important role in supporting individuals experiencing behavioral health crises. As a Care Coordinator at the Regional Crisis Stabilization Center (RCSC), you will work closely with a multidisciplinary team to help clients access services, coordinate care, and achieve positive outcomes. Since 1971, the Hampton-Newport News Community Services Board has provided the highest quality of behavioral health services to residents of the cities of Hampton and Newport News, Virginia, offering a comprehensive array of programs for both children and adults living with mental illness, developmental disabilities, and/or substance use disorders.
POSITION SUMMARY
The Care Coordinator for Crisis Stabilization supports timely, well-coordinated care within the Regional Crisis Stabilization Center (RCSC) by connecting individuals in crisis to appropriate community-based services and next-step supports. Key responsibilities include coordinating referrals and transitions of care; serving as a liaison between RCSC and community programs; supporting discharge planning and aftercare coordination; completing case management activities (including benefits applications) and accurate electronic record documentation; collaborating with internal/external providers; reviewing pre-screening evaluations for appropriateness; developing client service plans; providing direct crisis intervention support; and maintaining confidentiality and advocacy in accordance with policy. Required qualifications/skills: Master's Degree in Human Services. One (1) year experience providing service collaboration and coordination, psychoeducation, and behavioral health skills trainings to the client population assigned. Must be registered with the Board of Counseling to practice as a QMHP, or register as a QMHP - Trainee within 30 days of employment.
A TYPICAL WORK DAY
Your day follows a steady Monday-Friday schedule (8:30 a.m.-5:00 p.m.) within a multidisciplinary crisis setting. You'll start by reviewing current consumer needs and coordinating with team members to align next steps. Throughout the day, you'll stay in close contact with community programs and partner providers to ensure smooth handoffs after a crisis stay. You'll balance brief client interactions with time set aside for timely electronic documentation, benefits-related paperwork, and updates that keep the care plan moving forward. As situations change, you'll shift priorities, communicate clearly with stakeholders, and help maintain a consistent, client-centered approach to stabilization and follow-up.
