Assist the Director of Social Services with the timely delivery of
NOMNC & ABN
Notices as well as the expedited appeals process. Complete all appropriate sections of the MDS timely and accurately. Assist the Director of Social Services with Quality Improvement efforts, and regulatory compliance as it relates to NOMNCs, PASRR, Advance Directives, Care Plan meetings, and Ombudsman notification. Complete appropriate referrals to community agencies and resources available to meet the needs of patients throughout care transitions. Demonstrate and promote a positive, collaborative, and caring service excellence team environment.Qualifications Qualifications:
- High School Diploma or GED
- Some training or experience in working in a healthcare social service setting preferred
- Strong customer service, teamwork and communication skills.
