Job Description
Position Summary The OBAT Care Coordinator (QMHP-A) is responsible for coordinating and supporting comprehensive behavioral health and recovery services for individuals participating in Office-Based Addiction Treatment (OBAT) services. The Care Coordinator serves as a critical member of the multidisciplinary treatment team and assists individuals with accessing medical, behavioral health, recovery, and community support services necessary to maintain recovery and improve overall functioning. The QMHP-A Care Coordinator works closely with medical providers, nursing staff, therapists, CSACs, peer recovery specialists, and outside agencies to ensure continuity of care and support treatment engagement for individuals with substance use disorders and co-occurring mental health conditions. Services may be provided in office, community, hospital, telehealth, or other approved service settings in accordance with DMAS, DBHDS, and agency requirements. Essential Duties and Responsibilities Care Coordination & Case Management Coordinate behavioral health, medical, substance use, and community support services for individuals enrolled in OBAT services. Assist individuals with accessing housing, transportation, food resources, employment supports, insurance benefits, social services, and community-based resources. Monitor client engagement, appointment compliance, and treatment participation. Conduct outreach to individuals who miss appointments or are at risk of disengagement from treatment. Coordinate referrals to higher or lower levels of care when clinically indicated. Assist with discharge planning and continuity of care services. Collaborate with probation officers, hospitals, social services, primary care providers, pharmacies, and other treatment providers as appropriate. Clinical Support Services Assist with screenings, assessments, intake coordination, and treatment planning processes. Provide psychoeducation related to recovery, relapse prevention, coping skills, medication compliance, and community resources. Support medication adherence and recovery engagement efforts. Participate in multidisciplinary treatment team meetings and staffing. Monitor client progress and communicate concerns to the treatment team. Assist with crisis intervention and coordination of emergency services when necessary. Documentation & Compliance Complete all required documentation within established agency and Medicaid timelines. Maintain accurate, timely, and audit-ready records in the electronic health record system. Ensure documentation supports medical necessity and service compliance. Maintain confidentiality in accordance with HIPAA and 42 CFR Part 2 requirements. Participate in internal audits, supervision, and quality assurance activities. Maintain compliance with DMAS, DBHDS, ARTS, and agency standards. Collaboration & Team Participation Work collaboratively with OBAT prescribers, nurses, therapists, CSACs, peers, and administrative staff. Maintain ongoing communication regarding client needs, barriers, and treatment progress. Participate in staff meetings, trainings, and supervision sessions. Promote a person-centered, trauma-informed, and recovery-oriented treatment environment.
Qualifications Required:
Current Virginia Qualified Mental Health Professional - Adult (QMHP-A) credential. Minimum of one (1) year experience working in behavioral health, substance use treatment, MAT/OBAT services, case management, or community mental health. Knowledge of substance use disorders, co-occurring disorders, recovery principles, and behavioral health systems. Strong organizational, communication, and documentation skills. Ability to manage multiple tasks and maintain professional boundaries. Valid driver's license with acceptable driving record.Preferred:
Experience working within ARTS, MAT/OBAT, ACT, IOP, PHP, or community behavioral health settings. Experience with Medicaid documentation and service authorization processes. Knowledge of motivational interviewing, trauma-informed care, and harm reduction approaches. Experience using electronic health record systems. Physical Requirements Ability to travel throughout the community as needed. Ability to sit, stand, walk, bend, and lift up to 25 pounds occasionally. Ability to safely operate a motor vehicle. Work Environment Services may be provided in office settings, community locations, hospitals, homes, shelters, correctional settings, and other approved treatment environments. Position may involve exposure to behavioral health crises and unpredictable situations. Performance Expectations Maintain productivity and documentation standards established by the OBAT program. Demonstrate professionalism, ethical conduct, teamwork, and recovery-oriented care. Maintain compliance with all applicable federal, state, Medicaid, and agency regulations.
