at Northeast Kingdom Human Services in St. Johnsbury, Vermont, United States Job Description Bridge Program Care Coordinator If you're looking for an opportunity to do meaningful work supporting families in your community, join the passionate, community-minded team at Northeast Kingdom Human Services (NKHS). NKHS is seeking a dedicated and detail-oriented Care Coordinator to support families with children and youth (birth to age 22) who have developmental disabilities through our Bridge Program . In this role, you will provide high-quality service coordination designed to promote stability, connect families with critical resources, and support meaningful outcomes for the individuals and families we serve. This is a temporary position with the possibility of becoming permanent based on program needs and performance. This position plays an important role in helping families navigate complex systems of care while ensuring children and youth receive the services and supports they need to thrive. Key Responsibilities Conduct assessments to determine needs and service eligibility Develop individualized care plans that reflect each family's goals and support needs Coordinate and monitor services by collaborating with providers, schools, medical professionals, and community partners Assist families in navigating available resources and ensure timely access to essential supports Research available services and proactively identify resources to support informed decision-making Maintain accurate documentation and ensure compliance with program and regulatory standards To view full details and how to apply, please login or create a Job Seeker account
