NKHS is seeking a dedicated and detail-oriented Care Coordinator to support families with children and youth (birth to age 22) who have developmental disabilities through our Bridge Program . In this role, you will provide high-quality service coordination designed to promote stability, connect families with critical resources, and support meaningful outcomes for the individuals and families we serve. This is a temporary position with the possibility of becoming permanent based on program needs and performance. This position plays an important role in helping families navigate complex systems of care while ensuring children and youth receive the services and supports they need to thrive.
