The Cottages of Snohomish is searching for a new Resident Care Coordinator for our Assisted Living and Memory Care units. This is a great opportunity to serve our community and local seniors. RIGHT RCC
will be kind, caring, dependable and hardworking with a positive attitude and willingness to learn. Duties Coordinate and manage care services for individuals in need of support. Develop and maintain care plans based on needs and goals of residents. Assist residents with daily activities, including meal preparation, personal hygiene, and mobility support. Utilize behavior management techniques to promote positive behaviors and reduce challenging behaviors Collaborate with other professionals, such as social workers and healthcare providers, to ensure comprehensive care for individuals. Provide support in memory care programs, ensuring residents engage in meaningful activities that promote cognitive function. Conduct regular assessments of resident needs and adjust care plans accordingly. Lift and assist residents as needed while maintaining their comfort and safety. Document all care-related activities accurately in accordance with facility policies. Maintain accurate documentation of care provided and progress made by individuals Monitor and evaluate the effectiveness of care plans and make necessary adjustments Qualifications Previous experience in a similar role, such as direct support professional or care coordinator, is preferred Knowledge of behavior management techniques and systems is highly desirable Familiarity with care plans and the ability to implement them effectively. Strong interpersonal skills with a compassionate approach to assisting residents. Familiarity with home care services, memory care and regulations is a plus. Ability to lift and assist residents safely while adhering to best practices in caregiving. Excellent communication and interpersonal skills are essential for effective collaboration with individuals, families, and other professionals Excellent organizational skills with attention to detail to manage multiple tasks efficiently.
