Job Description
Rock River Community Clinic is a non-profit community health center that provides cost-effective and comprehensive health care services through clinics located in Johnson Creek, Whitewater and Watertown. We work diligently to ensure all community members have access to health care and serve as a safety-net clinic for families in need. The Care Coordination Assistant supports providers and care teams by coordinating referral services, specialty appointments, and access to community resources at all three locations. This role works directly with patients to schedule appointments, navigate insurance requirements, address barriers to care, and ensure timely communication between providers, specialists, and community partners.
Essential Duties and Responsibilities:
Care Coordination & Referrals Maintain ongoing tracking and accurate documentation of all referrals using the electronic medical record (EMR) and Referrals Tracking Spreadsheet to promote team awareness, continuity of care, and patient safety. Insurance & Authorization Support Utilize health insurance portals to verify patient demographic information, current insurance, and locate in-network providers. Collaborate with Patient Advocates to assist with financial assistance applications and enrollment processes. Review referral details, expectations, and preparation instructions with patients. Navigation & Advocacy Advocate on behalf of patients with insurance carriers or specialty providers to ensure timely access to services Serve as a point of contact for patients regarding referral and coordination needs Coordinate and schedule referral appointments as requested by providers. Manage outgoing and incoming referrals and ensure they are processed and addressed in a timely manner, Assemble and transmit appropriate clinical information to specialists according to referral guidelines. Ensure patient records are updated with specialist consult notes and community organization communications by utilizing external electronic medical record (EMR) access to obtain specialty clinic notes.

ENVIRONMENT & TRAVEL REQUIREMENTS
This position is based in a community health center environment that is generally clean, comfortable, and fast-paced. The role requires being on your feet with frequent movement around the clinic and office spaces with active engagement with patients and interdisciplinary care teams. A valid driver's license and reliable transportation are required.

POSITION REQUIREMENTS
Education Required:
High school diploma or
GED Preferred:
Associates degree or Bachelor's degree Licenses/Certifications CPR/BLS preferred Current Driver's License in the
State of Wisconsin Experience Required:
Experience in customer service and comfortable working with computers. Experience in customer service a primary care, FQHC, or community health setting strongly preferred
