The Behavioral Health Provider (BHP) delivers integrated, team-based behavioral health services within a primary care setting using the Primary Care Behavioral Health (PCBH) model, while also providing psychotherapy as part of the broader continuum of care. This role emphasizes flexible, patient-centered service delivery, including same-day access, brief interventions, and longitudinal therapy, based on patient need and program priorities, with a primary focus on brief, population-based care within the PCBH model. The BHP partners closely with primary care providers and the interdisciplinary team to address mental health conditions, substance use concerns, and behavioral factors that impact chronic disease management and overall well-being. Services are grounded in evidence-based practices and may range from brief, solution-focused interventions within primary care visits to more traditional psychotherapy, supporting both population health goals and individualized care. Essential Duties Collaborate with Primary Care Providers (PCPs) to deliver population-based, high-quality behavioral health care through brief (15-30 minute), evidence-based interventions, maintaining fidelity to the PCBH model Function as an integrated member of the care team by providing real-time consultation, participating in shared care planning, and supporting warm handoffs Provide psychotherapy using evidence-based treatment models for patients requiring longer-term or higher-intensity behavioral health support, as part of the clinic's continuum of care Maintain open-access scheduling to support same-day visits, brief follow-ups, and phone check-ins in a fast-paced, team-based primary care environment Utilize validated screening tools (e.g., PHQ-9, GAD-7, AUDIT-C) and apply measurement-based care to inform treatment decisions and track outcomes Coordinate care for patients requiring higher levels of behavioral health services, including referrals and collaboration with care coordination and external partners Complete timely, concise documentation and ensure accurate billing using appropriate psychotherapy and/or health and behavior codes Provide clinical supervision and training to students/trainees, including oversight of clinical care, documentation review, and professional development Support PCBH program development and staff education, including workflow optimization, training on warm handoffs, and integration best practicesTravel Requirements:
This position requires the ability to travel between all RRCC clinic locations and, as needed, within the community to support patient care, program operations, and team collaboration. Travel is typically local and may vary based on organizational needs. A valid driver's license and reliable transportation are required.Position Qualifications:

Master's degree inSocial Work, Counseling, Psychology, or Marriage and Family Therapy Licenses/Certifications Required:
APSW, LPC-IT, LMFT-IT, or equivalent required at minimumValid Wisconsin Driver's License Preferred:
Eligible for or currently licensed in the State of Wisconsin (LCSW, LPC, or LMFT) CPR/AED certificationExperience Required:
Minimum of 1 year of relevant clinical experiencePreferred:
Experience with integrated behavioral health or PCBH model strongly Experience in a primary care, FQHC, or community health settingSkills/Competencies Required:
