Job Description
NATURE OF WORK
A Service Coordinators duties include assessment of clients needs and strengths; planning and setting treatment goals and objectives with the client; evaluation of progress toward these goals; advocating on behalf of the client; linking/referring clients to needed community resources; providing supportive individual and/or group counseling to clients on caseload and in other areas within the agency; and crisis assistance/intervention. The Service Coordinator convenes the treatment team and acts as primary contact person for the client within the agency. The Service Coordinator serves as an information source and interfaces with external services ensuring quality of availability, continuity, flexibility, and integration of necessary resources and services.ESSENTIAL DUTIES
ofService Coordination:
Assess clients needs and evaluate progress toward treatment goals. Face-to-face contact with assigned clients at least once a month in his/her home and/or community. Use electronic conferencing technology, such as Zoom, Microsoft Teams, and Conference Call features on a Telephone to facilitate client interaction when face-to-face is not possible. Keep treatment plans and reviews current for the assigned caseload. Initiates, coordinates and facilitates interdisciplinary treatment team meetings for assigned caseload. Use clear communication skills with clients and other professionals. Link and refer clients to necessary resources and services to meet their individual needs. Maintain comprehensive knowledge of local and statewide substance use disorder resources. Completes and submits Electronic Medical Record documentation as required by Valley, OHFLAC, BMS, CMS, and other licensing agencies (Including Direct Support Progress Notes, Master Treatment Plans, etc.) completed accurately and timely and will accurately and appropriately reflect services the consumer(s) has received. Reviews consumers Electronic Medical Record (EMR) charts and documentation from other programs or service providers on a monthly basis to ensure coordination of services. Provide crisis services and intervention and/or assistance with consideration of the principles of client choice and least restrictive alternatives. Meets direct client service time (DCST) standards of the High Intensity Services Divisions Adheres to Valleys Standards of Conduct and Compliance. Serve on committees as assigned. Attend trainings as required.OTHER DUTIES AS ASSIGNED. MINIMUM QUALIFICATIONS
Ability to perform Essential Duties as outlined below. BA/BS in Human Service Field. Six months experience with SUD population. Ability to comply with all of Valleys Policies and procedures. Ability to comply with Clients Rights. Ability to comply with Valleys and Division departmental safety procedures. Ability to read, write and speak the English language. Valid Drivers License Ability to obtain and maintain CPR/First Aid, and MANDT Certifications.PREFERRED QUALIFICATIONS
