POSITION SUMMARY
The Community Coordinator plays a vital role in supporting patients by connecting them to healthcare and community resources. Working closely with medical providers, care teams, and community agencies, this role delivers short-term care coordination, education, and advocacy to improve patients' overall health and well-being. This position operates in both clinical and community-based settings.
ESSENTIAL FUNCTIONS
Provides a warm handoff in the clinic to provide patients with coordination care between mental health services, dental, and pharmacy Assist patients and families in understanding available programs and how they support health and well-being Identify and connect patients to resources addressing social determinants of health (housing, food, transportation, etc.) Develop and support individualized health and social service plans and goals Document patient encounters, care coordination efforts, and communications accurately in electronic records Maintain thorough patient files, including notes, assessments, releases, and reports Educate patients on appropriate healthcare utilization, including alternatives to emergency room use Coach patients and families to effectively navigate community resources and social programs Provide support and advocacy during medical visits and ensure understanding of referrals and care plans Coordinate care and follow up with patients, providers, and community partners Build and maintain relationships with social service agencies and community providers Assist patients with applications, scheduling, and access to services Facilitate communication among care teams, patients, and families Track patient progress and compliance with care plans Manage community or state programs (e.g., Ryan White, transportation services, LCCP programs) Participate in team and organizational meetings Maintain strict patient confidentiality in accordance with HIPAA guidelines
JOB REQUIREMENTS/EXPERIENCE
Education:
Bachelor's degree in health and wellness related fields of study. Degree in social work, nursing, wellness education, etc. preferable.
Knowledge, Skills and Abilities:
Strong communication and interpersonal skills Ability to build trust with individuals from diverse cultural and socio-economic backgrounds Knowledge of or willingness to learn community resources and social service systems Excellent organizational and documentation skills Ability to work collaboratively in a multidisciplinary team environment Experience in healthcare, social services, or community outreach preferred
Experience:
Two years' experience in community health, social work, direct patient care, mental health therapy or other related activities. Why Join HealthWorks?
