Description:
Leading MDs are seeking a Quality Coordinator to join the team. The Quality Coordinator for Leading MDs will be responsible for ensuring timely and accurate reporting of quality metrics for entities within the organization. This role involves analyzing and interpreting healthcare data, designing and implementing strategies to improve metrics and reporting processes along with the development of process improvement plans to close open care gaps, and HCCs while collaborating with interdisciplinary teams.?
Requirements:
Job Duties Chart preparation for quality opportunities and clinical opportunities in advance of patient visit to ensure open care gaps are addressed.? Collaborate with providers and staff to provide feedback and assist in addressing care gaps or resolving?discrepancies. Reconcile monthly quality and HCC data for reporting purposes. Develop and implement strategic weekly and monthly plans to achieve quality objectives, including gap closure and HEDIS measures Maintain knowledge and stay up to date on HEDIS reporting requirements. Strict adherence to all HIPAA guidelines and regulations. Conduct telephonic outreach for gap closure. Provide patient education on the importance of gap closure. Perform in-office point of care tests. Perform other related duties as assigned. Requirement Qualifications? Certified/Registered Medical Assistant or Certified Nursing Assistant (MA, CMA, RMA or CNA) One (1) + years' experience within a healthcare setting One (1) + years' experience with data and quality abstraction utilizing information from electronic medical records and other forms of information. One (1) + HEDIS/Stars experience. Proficiency in using Microsoft Office programs and navigating insurance portals. Ability to work in both independent and collaborative settings. Demonstrate time management skills and ability to adhere to project productivity timelines. Strong interpersonal and communication skills. Familiarity with HCC coding and recapturing. Knowledge of medical billing and
CPT/CPT II
coding standards. EMR experience, eClinical Works (ECW) preferred.
